How does melinda from ghost whisperer have a baby with Jim if Jim dies before shes pregnet?

I am so confuseed! I love this show but i dont understand how Melinda get pregante or if Jim will ever come back because he was awesome! And is Melinda the only person that sees Jim's face on sam's body?
